How do I fix Macintosh OS?

My friend have a old iMac thats running on Mac OS 9.2. It's not running through Mac OS X, It's just running on mac os. It will not boot up unless the extensions are off (by holding shift at startup). I don't know why and I don't want it to be like this. I don't have the restore installation disk for it. Please help me fix it.
